
Yolanda Smith ,RN, MSN, FNP-C
Yolanda is a Family Nurse Practitioner certified by the American Academy of Nurse Practitioners. She earned her Masters in Nursing/Family Nurse Practitioner degree from the University of Phoenix. Yolanda has over 24 years healthcare experience in Telemetry, Surgical, Ortho/Rehabilitation and Palliative Care, which has provided a foundation for her passion of nursing. She focuses on delivering holistic care and a positive healthcare experience for her patients.
Yolanda is a native of St. Louis, Missouri. She enjoys spending time with her two sons.

John Baca
Dr. Baca is the Founding physician of Triad Pain Management Clinic. He has focused his practice in the treatment of musculoskeletal, acute/chronic injuries and pain of the spine and extremities. His treating philosophy is to identify the “route cause” of the patient’s pain syndrome or injury. By properly diagnosing a condition and identify a pathway to healing and restoring function; he recognized that producing a positive and successful outcome involved a multi-disciplinary approach.
This philosophy established the multi-disciplinary evidence based model of care for Triad Pain Management.
Dr. Baca was raised in New Mexico; he attended New Mexico State University on an athletic baseball scholarship and graduated in 1994 with degrees in Biology and Health Care Administration. He went on to receive a degree in Anatomy and a Doctorate of Chiropractic Medicine, from Parker University in Dallas, Texas graduating Magna Cum Laude, in 1998. He entered private practice in 1998 and has since become a Diplomate in Pain Management by the American Academy of Pain Management and has been credentialed as a Board Certified Independent Chiropractic Examiner by the American Board of Independent Medical Examiners. He has extensive experience as a Manipulation Under Anesthesia (MUA) Certified physician holding privileges at several Arizona Surgical Centers. He has performed over 2500 MUA’s and proctored and trained physicians in MUA.
Dr. Baca believes in life-long learning. He stays abreast of the latest advancements and evidenced based research to support clinical outcomes in order to bring his patients the highest standard and quality care. Due to his experience and integrity; he serves Board positions for the Arizona Work Disability Prevention Association and for a local ambulatory surgery center (ASC) that is a Medicare approved facility and accredited by the Accreditation Association for Ambulatory Health Care (AAAHC). Dr. Baca is an avid sports fan; he enjoys “road-trips”; the “great outdoors” and loves spending time with his family.

Kevin Kerchansky
Dr. Kevin Kerchansky focuses his career on functional rehabilitation, biomechanics and human performance. He is a Board Certified Family Nurse Practitioner, and is also Board Certified by the American Chiropractic Rehabilitation Board. Dr. Kerchansky is certified by the American Board of Independent Medical Examiners, and is frequently hired as an expert to perform examinations of injured patients.He was born in Columbus, Ohio, and graduated from The University of Dayton in 1997 with a Bachelor’s of Science Degree in Pre-Medicine. He then attended Northwestern Health Sciences University, graduating with a Doctorate in Chiropractic Medicine. In 2020 he graduated from Herzing University and became a Family Nurse Practitioner.
In 2011, Dr. Kerchansky began lecturing nationally on the functional rehabilitation of injuries. His knowledge of rehabilitation led to speaking invitations at national symposiums for a variety of professional fields including chiropractors, physical therapists, attorneys, nurse case managers and insurance claims departments. Dr. Kerchansky has a passion for health and nutrition, is an avid golfer and enjoys going on adventures with his family.

Dr. Chris Walker
Dr. Chris Walker has a passion for health sciences and a strong desire to help people heal from medical conditions. This passion is what led him to become a chiropractor. He attended Arizona State University earning a Bachelor’s of Science in Kinesiology. He received his Doctorate of Chiropractic from Life Chiropractic College West.
Dr. Chris is an Arizona native. After receiving his doctorate, he moved back to the Chandler, AZ area to be close to family and provide chiropractic treatment to his home state. He has extra experience working with athletes, ranging from NFL combine athletes to collegiate men’s and women’s rugby players.
He is also a certified Webster Technique practitioner, specializing in chiropractic care for women during pregnancy and children. He uses his variety of skill to provide you with superior acute and chronic pain relief. He is an experienced family chiropractor providing care for patients of all backgrounds and ages.
When he is not taking excellent care of patients, he enjoys spending time with his wife, Michelle, and their daughters Reese & Rose. He also enjoys golfing and cheering on Arizona sports teams
